The Admission Nurse performs initial and ongoing evaluations of hospice patients to identify physical, psychosocial, and environmental needs. They implement and revise care plans while communicating critical changes in patient condition to the medical team.
Candidates must be a graduate of an accredited nursing program with a current, unrestricted state RN license and at least one year of clinical nursing experience. A valid driver's license, dependable vehicle, and the ability to travel 50% of the time are required.
