The Clinical Team Manager oversees patient care services, coordinates referrals, and ensures the development and implementation of individualized patient plans. They also manage direct care staff, conduct OASIS reviews, and facilitate communication between physicians, patients, and the care team.
Candidates must hold an Associate degree in a Nursing-related field and a current, unrestricted RN license. The role requires at least 2 years of home health experience and 1 year of supervisory experience, along with proficiency in OASIS assessments.

What is Required?
The base salary for this position is $90,000 to $110,000 annually, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, internal equity and production points.
This role may also be eligible for additional compensation, which could include performance-based bonuses, commissions, on-call pay, or other incentive earnings. These components are not guaranteed and depend on individual and/or team performance.
Full compensation details, including total rewards and applicable benefits, will be discussed during the interview process. You will report to the Administrator or Branch Director.
