The Physical Therapist will provide direct patient care in home settings, evaluating functional needs and designing personalized treatment plans. They will also document patient progress, collaborate with other specialists, and provide leadership to support staff to ensure quality outcomes.
Candidates must be graduates of an APTA-approved physical therapy program and hold a current, unrestricted PT license. At least one year of experience in a healthcare facility and the ability to travel within the coverage area with a reliable vehicle are required.
