The Service Coordinator provides professional assessment, coordination of mental health services, and advocacy to support individuals in achieving a safe and healthy community life. They are responsible for maintaining accurate service records and linking clients to natural and specialized community supports.
Requires a Bachelor's degree in Social Work, Psychology, or a related field with at least one year of experience in human services. Candidates must possess a valid PA driver's license and obtain necessary state criminal and child abuse clearances.
Service Coordinators play a critical role in fulfilling the mission of TCV by providing professional assessment, coordination of mental health services and supports, service planning, linkage, referral, and advocacy services required for a safe and healthy community life based on the individual’s level of needs. The incumbent works as a member of a treatment/service team in order to promote individuals’ mental health recovery. We are looking for people who believe in a strengths-based approach and use positive practices to help guide their everyday activities. The Service Coordinator position is designed to:
