Position Summary

The Medical Case Manager is responsible for providing those living with HIV a comprehensive continuum of services through critical linkages with community resources. This includes the provision of treatment adherence counseling to ensure readiness for, and adherence to, complex HIV treatments. This position entails an assessment of service needs; development of a comprehensive, individualized service care plan; coordination of services required to implement the plan; client monitoring; and re-evaluation and adaptation of the plan over the life of the client. Contacts with clients will be face-to-face at medical, home, office, and hospital visits. 

This position reports to the Manger of Care Services This is a(n) HYBRID position.

The Medical Case Manager applies the principles of the organization’s signature service delivery approach, Primary Client Model (PCM™), by promoting a positive employee/client-focused experience that is appreciative of the unique strengths, values and needs that each individual brings; and by using data to inform and drive decision making. Attention is given to compliance, quality assurance, and best practices as they relate to the role.

Essential Functions

  • Represents Congreso’s mission, values, and commitment to diversity, equity, inclusion, and belonging and will be an ambassador of our work.
  • Maintains a caseload of people living with HIV as required by contractual standards. 
  • Conducts regular face-to-face medical visits, home visits and hospital visits.
  • Conducts and completes comprehensive assessment for each client.
  • Assesses the client’s needs and completes an initial and quarterly Service Care Plan.
  • Conducts in-depth treatment adherence counseling for clients. Specifically, but not limited to.: Knowing what medication the client is taking; When the client is taking those medications; Possible side effects; Knowing and understanding the client’s CD4 cell count and Viral Load results.
  • Responsible for monitoring HIV lab results for each assigned client with the goal of securing stable health.
  • Responsible for working directly with SSI, welfare, health insurance/medical coverage and related social services that are directly related to the client’s health.
  • Completes and submits client referrals for support services: food/clothing, financial benefits, transportation, legal, medical, living situation, substance abuse, support systems, mental health, cultural/language.
  • In collaboration with the Administrative Team: identifies problems that may hinder access to services and advocates for solutions to these problems.
  • Responsible for completing and maintaining clear, concise records and documentation for each client as required by the funding agency.
  • Maintains daily, weekly, and monthly service reports and submits these to supervisor as per the deadlines.
  • Ensures continuous communication with the client’s primary care physician, mental health provider, drug and alcohol counselor and any other provider(s) that is servicing the client with the goal of achieving and maintaining Viral Load suppression. 
  • Assists the Supervisor in establishing and maintaining affiliated agreements with other community-based organizations that serve at-risk minority individuals, to increase access to case management services. This includes maintaining relationships with other medical providers.

  • Demonstrates knowledge and understanding of the community needs and demographics.
  • Ability to understand, acknowledge, and assess client needs to provide a timely, trauma informed approach in service.
  • Ensures the appropriate handling of sensitive information and understanding for issues such as confidentiality, domestic abuse, child abuse, and mandated reporter requirements.
  • Adheres to all Congreso, departmental, programmatic, and security policies, procedures, and guidelines.
  • Attends all Congreso trainings and in-services as required, including relevant professional groups to maintain professional, requisite knowledge and licensure.
  • Attends, participates, and facilitates relevant staff meetings to promote communication and execution of goals.
  • Ability to effectively use standard office equipment, including a variety of common software programs such as Office365, UNIDAD, and other customized databases.
  • Follows through on assignments, works in a timely manner, within established deadlines, and completes special projects specific to the function of the department, as needed or as directed by supervisor.
  • Demonstrates ability to work independently and collaboratively with diverse groups, in a compassionate, courteous, and respectful manner that fosters an inclusive environment.
  • Strong written and verbal communication skills including effective and respectful communication with individuals and groups.
  • Bilingual (Spanish/English) written and verbal skills required. (*May be changed to a plus if staffing permits). 

Supervisory Responsibilities: 

This position has no supervisory responsibilities. 

 

Minimum Experience, Education, and Qualifications

Minimum Education: 

Bachelor’s degree in social work, public health, counseling, psychology, nursing, or related field required, Master’s degree preferred. 

Minimum Experience: 

1-3 years’ experience required.

 

Certification/License: 

Valid driver’s license preferred.
