The Medical Case Manager provides comprehensive services and treatment adherence counseling for individuals living with HIV. Responsibilities include conducting assessments, developing individualized care plans, and coordinating with medical providers and community resources to ensure viral load suppression.
A Bachelor's degree in social work, public health, counseling, psychology, or nursing is required, with a Master's degree preferred. Candidates must have 1-3 years of experience and bilingual proficiency in Spanish and English.
The Medical Case Manager is responsible for providing those living with HIV a comprehensive continuum of services through critical linkages with community resources. This includes the provision of treatment adherence counseling to ensure readiness for, and adherence to, complex HIV treatments. This position entails an assessment of service needs; development of a comprehensive, individualized service care plan; coordination of services required to implement the plan; client monitoring; and re-evaluation and adaptation of the plan over the life of the client. Contacts with clients will be face-to-face at medical, home, office, and hospital visits.
This position reports to the Manger of Care Services This is a(n) HYBRID position.

Bachelor’s degree in social work, public health, counseling, psychology, nursing, or related field required, Master’s degree preferred.
1-3 years’ experience required.
