Responsibilities
The Field LVN assists the RN Case Manager in coordinating and implementing the patient's plan of care. Responsibilities include administering medications, documenting clinical notes, and managing patient symptoms while participating in an on-call rotation.
Requirements
Candidates must hold a valid Texas Licensed Vocational Nurse (LVN) license and a driver's license with reliable transportation. At least one year of LVN experience in an acute care setting is required, with hospice or oncology experience preferred.

$2,500 Sign-on bonus
POSITION SUMMARY
Assist the RN Case Manager in the plan of care for the patient.
ESSENTIAL FUNCTIONS
Assists RN in case management of total plan of care of patient/support persons by coordinating with team members and communicating with IDG in accordance with organizational policies, procedures, and regulations that define the scope of practice.
Complete, maintain, and submit accurate and relevant clinical notes regarding the patient’s condition and care given. Record pain/symptom management changes/outcomes as appropriate.
Administer medications and treatments as prescribed by the physician.
Document nursing care and significant interactions with patients, support persons, and team members.
Assess patient/family response to care and participate in follow-up with bereaved families with various types of contact.
Manage assigned patients equally, regardless of severity, contagion, or complexity of diagnosis.
Participate in on-call rotation
POSITION QUALIFICATIONS
Licensed Vocational Nurse licensed by the State of Texas, required
Hold a valid driver’s license and have a dependable means of transportation
1 year of nursing, hospice, or oncology experience preferred
1 year LVN experience in an acute care setting required.
Proficient skills using a computer are required; Microsoft Office suite experience preferred
