Responsibilities
The Health Care Coordinator oversees the overall healthcare and well-being of individuals in residential group homes and independent apartments. Responsibilities include conducting staff training on treatment protocols, performing medical file audits, and visiting residential homes and hospitals.
Requirements
Candidates must hold a current Massachusetts Registered Nurse (RN) license and have 3 to 5 years of experience in community health, preferably with adults with intellectual disabilities. A valid driver's license and the ability to pass a comprehensive background check are required.

The role!
Position: Residential RN Healthcare Coordinator Location: Wakefield, MA Pay rate: $68,640 annual salary Hours: Full-time, 30 hours per week Schedule: This position will typically operate on a Monday – Friday schedule; may be required to work evenings and weekends in emergency healthcare needs; must have flexibility as schedule may vary to meet needs of agency and the Individuals we support

Summary & Responsibilities
The Health Care Coordinator will be responsible for the oversight of the overall healthcare and well-being of individuals who reside in our Residential Program group homes and independent apartment settings. We are looking for someone with a strong background and experience in working with adults with Developmental and Intellectual Disabilities.
Staff training on specific treatment protocols
Consultation for individual medical needs
Perform MAP trainings and recertifications
Perform MAP and medical file audits
Residential home and hospitalization visits as needed
Attend administrative meetings and complete related paperwork
Qualifications
Must have a current Massachusetts license as a Registered Nurse (RN).
Must have at least 3 to 5 years’ experience in a community health setting; preferably with adults with developmental and intellectual disabilities.
Certified MAP Trainer is a plus but training is also available
Successful candidates will be required to complete a background check including references, CORI, and fingerprinting.

Valid Driver’s license, reliable transportation, and acceptable driving record
