GENERAL RESPONSIBILITIES 

Provides oversight to Black Family Development, Inc.’s Triage Department, Intensive Care Coordination with Wraparound with expertise in Evidence-Based Practices. Ensures fulfillment of CCBHC, DWIHN, and other funding bodies requirements in the areas of: personnel, operations, claims submission, credentialing, implementation timeframes, documentation, and all other governing requirements. Assist in the development of needed programs and services within the organization, or with other service providers.

DUTIES AND RESPONSIBILITIES

  • Ensures that all persons served are treated with respect and dignity. 
  • Provides supportive coordination to include the nine (9) core service areas: 
    • Crisis Services
    • Treatment Planning
    • Screening, Assessment, Diagnosis & Risk Assessment
    • Outpatient Mental Health & Substance Use Services
    • Targeted Case Management
    • Outpatient Primary Care Screening and Monitoring
    • Community-Based Mental Health Care for Veterans
    • Peer, Family Support & Counselor Services
    • Psychiatric Rehabilitation Services
  • Establishes compliance strategies to uphold service limitation dates, billing submission time frames, and administrative due dates. 
  • Provides supervision to ensure family-centered treatment is rendered using evidence-based practices.
  • Responsible for the direct management and coordination of the care coordinators, including administration, staff supervision, monitoring of program services, trainings, completing required reports, and maintenance of records.
  • Monitors and assures quality service provision and ensures that consumers’ needs are met through review of documentation, incident report reviews, review of Satisfaction Surveys and other evaluations.
  • Completes quarterly reports related to service provision, barriers and needs for Triage, PMTO, Intensive Care Coordination with Wraparound, and SED-Waiver.
  • Attends Parent Management Training – Oregon (PMTO) administrative meetings and trainings with the Michigan Department of Community Health, or regional gatherings facilitated by an MDCH-approved designee.

· Conducts monthly Community Team meetings and maintains memberships. 

· Supervise and assist agency’s interns in attainment of goals and objectives as a part of their educational plan. 

· Works with universities’ liaisons to discuss and evaluate interns' progress in field placement. 

  • Provides routine staff trainings on mental health, PMTO, quality improvement and/or topics and compliance issues.
  • Assists with crisis resolution with 24/7/365 availability.
  • Timely and accurate submission of monthly billing, quarterly reports, and all other funder-and BFDI-required reports.
  • Promptly implements program corrective actions as needed.
  • Ensures the integrity of contractual requirements. Promptly implements program corrective actions as needed.

  • Participates as a member of at least one (1) staff committee
  • Performs other duties as assigned. 

COMPETENCIES

  • Strong understanding of developmental disabilities, chronic diseases, and social determinants of health.
  • Proficient in screening adults, youth, and families to assess emergent needs.
  • Demonstrates effective leadership and decision-making skills.
  • PMTO Certification and adherence to coaching requirements.
  • Provides support for interns' educational plans.
  • Expertise in Intensive Care Coordination following Wraparound certification guidelines.
  • Skilled in implementing Evidence-Based Practices.
  • Experienced with BFDI electronic record systems.
  • Knowledgeable in Crisis Intervention strategies.
  • Well-versed in agency admission decision criteria.
  • Familiar with CCBHC, DWIHN, ICCW, PMTO, and BFDI policies and procedures.

Requirements

Minimum master’s degree in a human service field and supervisory experience. Strong understanding of cultural competency and family-centered services with the target population. Treatment/prevention knowledge of client population and treatment strategies; experience in family casework; experience completing comprehensive assessments, data collection, and report writing. Knowledge of strength-based focused practices, assessment tools, and community resources. Computer literate in both word processing and database software programs. Willing to work a flexible schedule and maintain 24-hour/7-day accessibility
