The RN Case Manager performs patient assessments and collaborates with a care team to implement individualized plans of care. Responsibilities include managing pain and symptoms, educating families, and handling on-call services.
Candidates must have at least one year of nursing experience and a current RN license for the assigned state. A valid driver's license, reliable transportation, and CPR certification are also required.
