The RN case manager performs patient assessments and collaborates with a care team to implement individualized plans of care. They are responsible for documenting progress, managing pain and symptoms, and educating patients and their families on care techniques.
Candidates must have at least one year of nursing experience and a current RN license for the assigned state. Additional requirements include CPR certification, a valid driver's license, reliable transportation, and liability insurance.
