Job Description Summary:
The Ambulatory and Outpatient Registered Nurse (RN)/Registered Dietician (RD), provides high-quality, patient-centered care through evidence-based practice, clinical expertise, and compassionate care delivery in an ambulatory care setting. The RN/RD coordinates and facilitates education of the diabetes patient outpatient population, by acting as a liaison between patients, families, and medical staff.

We’re Looking For:
Bachelor's degree in Nutrition (RD) OR Registered Nurse (RN) license issued by the state in which teammate practices.
2-3 years nutrition experience with exposure to diabetes patients OR 3-5 years in clinical specialty.
CDCES certification
Experience with insulin pump therapy
Knowledge & understanding of, child growth & development, normal patterns of adolescent behavior, and signs of normal aging
What You’ll Do:
Schedule: Monday – Friday: 8a-4:30p (May change based on clinic/patient needs) Position is based in Merrionette Park with occasional travel to Orland Park
As a skilled and compassionate Diabetes Educator, you will play a pivotal role in providing organizing, educating and scheduling patients for the Outpatient Diabetes Self-Management Program.
Participating in instructing outpatient groups and one-on-one sessions
Serve as a resource/clinical expert for identified diabetes inpatients.
Reinforces education to patients / families as requested.
Provides hospital staff with adequate supplies of Diabetes educational materials.
Plan and/or participate in any diabetes-related activities offered to the community, e.g. glucose monitoring, screening, lectures.
Using evidence-based practice, clinical decision making, compassion, and skills communication while leading efforts to create the safest patient environment and the best patient experience across the continuum.

Our team consists of experienced endocrinologists, physicians in fellowship training, physician residents, registered dieticians/diabetic educators, pharmacists, nurses, medical assistants, and administrative professionals who work collaboratively to deliver comprehensive preventive, chronic, and acute care services.

Physical Requirements and Working Conditions
Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday.
Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies.
Must be able to: push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present.
Must have functional speech, vision, hearing, and touch with ability to use fine hand manipulation skills for IV insertion and other procedures/functions.
Maneuver foot pedals on carts or machines.
Perform physical safety interventions such as patient restraint and verbal de-escalation, if needed.
May be exposed to chemical and hazardous waste as well as blood and body fluids and communicable disease. Therefore, protective clothing and equipment must be worn as necessary.
Must be able to respond quickly to changes in patient and/or unit conditions.
Physical Abilities Testing may be required.
Additional department specific physical requirements may be identified for unique responsibilities within the department by the nurse leader.
