Trumen Physicians + Associates is seeking an experienced and compassionate Nurse Practitioner to join our Trumen Transitional Care Team (TTC) on a part-time, supplemental basis in San Antonio, TX. This unique opportunity is ideal for providers looking to make a meaningful impact in patient care while enjoying unmatched flexibility and autonomy.


About the TTC Program:

Trumen’s Transitional Care Program supports adults with acute or chronic conditions as they move from the hospital or skilled nursing facility back to their community setting—such as their home or assisted living. The goal is to prevent avoidable readmissions by ensuring a smooth, medically supported transition.

As a TTC Provider, you will:

  • Travel to patients' homes shortly after discharge

  • Reconcile medications

  • Review and adjust treatment plans

  • Manage acute and chronic conditions

  • Communicate with patients’ family and primary care providers when needed

You will work closely with a dedicated TTC Coordinator to build your monthly schedule based on your availability and travel preferences.

Responsibilities:

  • Conduct home visits for patients recently discharged from hospitals or SNFs

  • Perform medication reconciliation and health assessments

  • Provide short-term management of chronic and acute conditions

  • Educate patients and families on care plans and red-flag symptoms

  • Coordinate care and updates with primary providers and specialists as needed

  • Complete documentation accurately and in a timely manner using EMR


Qualifications:

  • Active Texas Nurse Practitioner license

  • Minimum of 1 year of NP experience (transitional care, home health, geriatrics, or post-acute preferred)

  • Strong interpersonal and critical-thinking skills

  • Comfort working independently in a home-based setting

  • Reliable transportation throughout San Antonio
