The LPN Visit Nurse will provide professional nursing care, conduct patient assessments, and implement individualized care plans within a hospice setting. Candidates must hold an active LPN license and have graduated from an accredited school of nursing. Previous clinical experience in hospice or home health is preferred, along with strong assessment skills and a valid driver's license for local travel.

As an ECT (Extended Care Team) Nurse at Transitions Hospice, you're the reinforcement — the skilled, composed clinical presence that steps in to ensure no patient need goes unmet.
This isn't a role for someone who needs a rigid routine. It's a role for an adaptable, experienced RN who thrives in varied environments, thinks on their feet, and finds deep satisfaction in knowing that when you showed up, it made all the difference.
As a Transitions Hospice ECT Nurse, you'll work as a vital support member of the interdisciplinary care team — stepping in across cases to assess, treat, and communicate patient needs with speed and clinical precision. Your responsibilities include:
