The Care Manager-RN coordinates patient progression through the continuum of care by managing discharge planning and patient flow. They perform comprehensive assessments and collaborate with interdisciplinary teams to ensure cost-effective, safe, and high-quality transitions of care.
Requires an Associate's Degree or Diploma in Nursing and a valid RN license, with 2 years of clinical experience. BLS certification is required within 60 days of hire, while a BSN and Case Management certification are preferred.
A successful candidate's actual pay rate will be based on several factors including relevant experience, skills, training, certifications and education.
Associate’s Degree or Diploma in Nursing is required.
Bachelor’s Degree in Nursing is preferred.
2 years of clinical experience is required.
Prior case management experience is preferred.
Quality/Process Improvement experience is preferred.
License as a Registered Nurse (RN) in the state of practice is required. Wisconsin: RNs may practice with a valid multi-state RN license as outlined in the Nurse Licensure Compact rules.
Case Management certification (ACM, CCM, RN-BC) is preferred.
Basic Life Support Certification (BLS) is required within 60 days of hire.
