The LPN provides one-on-one patient care in home settings, focusing on medication management, wound care, and safety. They are responsible for coordinating care, reporting condition changes to RNs, and educating patients and their families.
Candidates must hold an active LPN license in the State of Minnesota and a current CPR certification. A valid driver's license, reliable transportation, and preferably two years of LPN experience are required.

Hours: Full-time | Days, Monday - Friday | There will be a weekend and holiday rotation. Work approximately one weekend every 4-6 weeks and 2 holiday's per year. The weekend and holiday rotation is dependent upon staffing levels.
Territory: This position will work in the field helping patients in their home setting. Our office is in Roseville, MN and the general territory for this position would be the southwest metro.
