Minimum Hiring Annual Salary:

$52,612.00

Maximum Hiring Annual Salary:

$56,821.00

To attain the maximum annual salary, employee must meet all minimum qualifications plus:

• 5 (five) years of additional applicable work experience and/or degrees higher than minimum qualification degree. Degrees higher than the minimum qualification degree can be used in combination with additional work experience.
• A combination of related education, on-the-job work experience, certifications and/or licenses that results in a candidate successfully meeting the minimum qualifications of the position, may be considered.

The stated salary amounts are not guaranteed in the event employment ends with Aims during any fiscal year.

Job Description:

The CARE Case Manager provides non-clinical student case management, in the moment solutions, and resource identification, referrals, and follow up to resources, and basic-needs support for struggling students. The CARE Case Manager works directly with students to customize and implement non-clinical interventions for students seeking resources and for students who have challenges with their health, housing, finances, academics, behavior, safety and other aspects of their general welfare. The case manager provides individualized care and services so students can meet their goals, including intake, needs assessment, in the moment support, service planning, and monitoring/evaluation of identified goals. This position will develop in-depth and ongoing relationships with students in order to increase supports and overall success. In conjunction with counseling services and with the SAIL Office, the CARE Case Manager will develop and implement educational, outreach, and preventative programs for overall student well-being. Occasional travel to satellite campuses may be required.

Non-Clinical Case Management: 50%

• Provide individualized care and case management services so students can meet their goals. This includes intake, needs assessment, resource management, service planning, and monitoring and evaluation.
• Teach students how to problem-solve by identifying alternative options as they relate to their life and career goals.
• Liaison with faculty when appropriate about student progress and student situations that could impact academic performance.
• Keep accurate log notes that reflect the discussion and essential information conveyed to students.
• Refer students to appropriate resources on campus based upon their needs (e.g. orientation, disability access, etc.).
• Refer students to appropriate community resources based on their needs
• Respond within 48 hours during regular working hours to student phone calls and email inquiries.
• Establish working relationships/partnerships with community agencies, services, and resources to meet students’ basic needs.
• Maintain information and network for campus and community resources.
• Provide initial contact for students who walk into the CARE and counseling office seeking services.
• Problem solve and develop plans to meet students’ needs, including ongoing support and comprehensive wrap around services.

Prevention & Wellness Programming: 25%

• Establish educational materials and support groups on campus to help students develop coping strategies for common basic needs.
• Develop referral resource information.
• Develop and conduct training sessions for faculty and staff on how to work with students who have basic needs on all campuses.
• Execute wellness tactics on the college strategic plan.
• Provide programming opportunities for students related to wellness.
• Work with the Director to develop an annual CARE program training series for faculty and staff.

Networking: 20%

• Establish and maintain close working relationships with a variety of Aims departments, including, Student Disability Center, TRIO, Center for Diversity and Inclusion, Registrar, Office of Financial Aid, Tutoring Center, Career Service, Transition Center, and other departments that may be helpful in meeting students’ needs and academic goals.
• Develop and maintain strong collaborative relationships with community and campus agencies. This may include participating in community workgroups and activities related to basic and mental health needs.
• Outreach to identify community resources and build relationships with community resources to facilitate an effective continuum of care.

Minimum Qualifications

• Bachelor's Degree in a Human Services field such as Social Work, Psychology, Counseling, Public Health, Human Relations, Higher Education, Student Affairs Administration, or another closely related degree.
• Two (2) years of professional experience working in social work, case management, student conduct, student services, psychology, health care coordination, counseling, human services, and/or otherwise coordinating services for students/clients or an equivalent combination of education and/or work experience.
• Proficiency in Microsoft Office.
• Assist students in distress and/or crisis

Preferred Qualifications

• Case management and/or coordinating services for students in a higher education setting.
• Bilingual English/Spanish
• Experience with student information systems, such as Workday, and conduct/case management databases, such as Maxient.
• Experience making professional presentations to groups of up to 50 people.
• Ability to manage and coordinate several programs, activities, and services simultaneously.
• Experience working with a diverse student population (i.e. post-traditional students, students of color, Veterans, first generation college students etc.) from underserved or underrepresented backgrounds in the higher education or human services environments, specifically in a social work capacity.
• Experience with Data collection and analysis in order to develop informed services.
