Conduct face-to-face assessments of vulnerable populations and collaborate with healthcare providers to create appropriate treatment plans. Perform quality assurance competency reviews and provide training and mentorship to new staff.
Requires a Bachelor's degree in healthcare, a current unrestricted DC RN license, and at least four years of healthcare experience. Experience in human services, disability care, and level-of-need assessments is strongly preferred.

General Summary
This position is responsible for face-to-face assessments of vulnerable populations in the District of Columbia. Persons being assessed reside in the community or in facilities. The incumbent may also complete review decisions regarding services needed for the person(s) to be successful in the community. This position may also serve as a mentor or preceptor for new staff, assisting in the formal training of assessors and completing quality assurance competency reviews. Additionally, the incumbent may be involved in process improvement initiatives.

This may be a home-based position that will primarily travel to cover a geographic area. As business needs and assessment volumes vary, additional travel outside of the primary geographic area may be required. Up to 95% local travel may be required.
