About Waymark

Waymark is a mission-driven team of healthcare providers, technologists, and builders working to transform care for people with Medicaid benefits. We partner with communities to deliver technology-enabled, human-centered support that helps patients stay healthy and thrive. We’re designing tools and systems that bring care directly to those who need it most—removing barriers and reimagining what’s possible in Medicaid healthcare delivery.

Our Values

At Waymark, our values are the foundation of how we work, grow, and support one another:

  • Bold Builders: We tackle the toughest challenges in care delivery by harnessing the power of community and technology.
  • Humble Learners: We seek feedback, embrace diverse perspectives, and welcome challenges to our assumptions.
  • Experiment to Improve: We use data to inform decisions and continuously assess our performance.
  • Focused Urgency: Our mission drives us to act swiftly and relentlessly in pursuit of meaningful results.

If this resonates with you, we invite you to bring your creativity, energy, and curiosity to Waymark.

About this role

At Waymark, our data science work is grounded in a specific and meaningful challenge: using claims, EHR, and care worker data to help community health workers, social workers, pharmacists, and care coordinators deliver better care to Medicaid patients.

As a Data Scientist you will contribute to building the models and tools that these care teams rely on — risk models, care gap prediction, and ML/AI applications including LLM-based tools that translate raw clinical data into actionable guidance. You will work closely with senior data scientists and engineers to learn how ideas move from prototype to production, and will be expected to take on increasing ownership as you develop in the role.

This is a position for someone who is technically strong, eager to learn healthcare data, and motivated by the idea that their work will directly affect the care of low-income patients.

Key Responsibilities

  • Build and validate ML and AI models on claims and EHR data for care delivery use cases including risk stratification and care gap identification, under guidance of senior data scientists
  • Support the development of LLM-based tools and AI applications for care team workflows
  • Develop and maintain Python code for data processing, model development, and ML pipelines, following software engineering best practices including version control, code review, and testing
  • Collaborate with engineering, product, and analytics teams to understand requirements and translate them to technical solutions
  • Develop Healthcare Subject Matter Expertise: Show a strong interest in healthcare data structures, quantitative methods, and data science applications in healthcare

Minimum Qualifications:

  • A Master’s degree in Data Science, Computer Science, Statistics, or a related field.
  • Python Proficiency: 3+ years of hands-on Python experience including academic and project work — demonstrated through a GitHub portfolio or equivalent
  • Solid foundations in statistical and machine learning methods, including classical ML and an understanding of modern AI and LLM applications
  • Strong SQL skills and comfort working with large, messy real-world datasets
  • Experience working with Git and collaborating in a team codebase
  • Strong communication skills and ability to work closely with cross-functional stakeholders

Preferred Qualifications:

  • 1-2 years of industry data science experience
  • Exposure to healthcare data (claims or EHR) through coursework, research, or work experience
  • Familiarity with ML experimentation practices — experiment tracking, model validation, reproducible workflows
  • Genuine curiosity about healthcare and its intersection with technology

Hiring Range

Waymark is a team of healthcare providers, technologists, and builders whose mission is to bring the best healthcare to people with Medicaid benefits. Guided by the communities we serve, we bring support and technology-enabled care to help primary care providers keep Medicaid patients healthy. We are building the tools and designing an approach to enable care to reach the patients who can benefit most.

Our core values embody the essence of what makes Waymark a unique team today, and what we look for, nurture, and sustain as a team.

  • We are bold builders, believing that the greatest challenges in care delivery can be solved when we harness the power of community and technology.
  • We are humble learners, seeking feedback and perspectives different from our own, and welcome challenges to our conclusions.
  • We experiment to improve, actively seeking data to inform decisions and to assess our own performance.
  • We act with focused urgency, our commitment to our mission drives us to tirelessly pursue results.

If this vision resonates with you, we hope you consider bringing your creativity, your energy, your curiosity to Waymark.

About this role

As a Clinical Pharmacist Manager for the California team, you will be key to our efforts to provide better care and support to vulnerable populations. We are looking for a Clinical Pharmacist Manager with experience working with vulnerable populations; collaborating with a multidisciplinary team including Community Health Workers, Care Coordinators, and Therapists; supporting patients through direct patient outreach, medication adherence and chronic disease education, and assistance in obtaining medications and durable medical equipment after initial rejection; and using analytics dashboards to proactively engage with patients who may be at high risk for adverse drug events and/or avertable acute care events. The Clinical Pharmacist Manager will supervise and support pharmacists and pharmacy technicians working to address patient barriers to medication access and identify patients who can benefit from Waymark’s assistance.

Key responsibilities
  • Provide telephonic and/or video and/or SMS-based outreach, engagement, and education of vulnerable patients in the Medicaid program in California
  • Work with a multidisciplinary team to provide care management services that ‘wrap around’ primary care provider practices; team members include Community Health Workers, Care Coordinators, and Behavioral Health Therapists
  • Support patients and providers around transitions of care
  • Provide clinical services that support day-to-day operations and safe practices of the program
  • Participate in QI activities to assure quality, cost-effectiveness, and ROI
  • Utilize analytics dashboards that help you proactively outreach patients who receive medication and/or equipment rejections, admissions/readmissions, or who may benefit from medication adherence counseling
  • Lead and mentor pharmacists and/or pharmacy technicians

Minimum qualifications

  • PharmD with advanced practice license in good standing in California
  • Completion of accredited PGY1 residency training in related discipline
  • 5+ years of experience in pharmacy practice including delivery of interventions/direct patient care in an ambulatory setting among vulnerable populations
  • Experience in multi-disciplinary care management
  • Positive referrals from prior colleagues

Preferred qualifications

  • Completion of PGY2 residency training in related discipline (ambulatory care, geriatrics, pharmacotherapy, internal medicine)
  • Strong analytic skills and ability to navigate dashboards and registry data for proactive outreach campaigns including for HEDIS gaps in care
  • History of experience with medication management through collaborative practice agreements with primary care providers
  • BPS Board Certification Preferred (prefer BCPS and/or BCACP)
  • Experience serving vulnerable populations with a skill set including motivational interviewing, harm reduction, and behavioral health
  • Leadership experience supporting other pharmacists, pharmacy technicians and/or other care team members

Waymark is an AI-first community clinic for Medicaid. For people enrolled in Medicaid, existing care models are reactive, inaccessible, and disconnected from the social needs that drive most avoidable hospitalizations and ER visits. Waymark is building something different — a clinic that starts with the patient, not the hospital. Our approach has been shown to reduce avoidable hospitalizations by 48% and ER visits by 20%, generating $2,347 in savings per patient per year (NEJM Catalyst, 2024), with a 35-percentage-point improvement in HEDIS gap closure compared to traditional methods (npj Digital Medicine, 2025). Today, we manage over $800M in claims across some of the nation's largest and most forward-thinking Medicaid health plans and health systems. These results are driven by our proprietary, peer-reviewed AI platform. Predictive models identify rising-risk patients before they reach a crisis. Clinical decision support ensures the right intervention at the right time. And a patient engagement layer keeps people connected to their care around the clock. This technology enables our multidisciplinary care teams — community health workers, pharmacists, therapists, and care coordinators — to find patients where they are and provide the clinical services and social support that keep them healthy. Waymark is the Medicaid clinic of the future: patient-centered, AI-powered, and community-based. Join us as we build it.
