Assist residents with social and emotional needs while supporting the development of care plans. Coordinate resources and referrals to advocate for the well-being and rights of residents and their families.
Preferred experience in social work, healthcare, or a related field. Candidates must possess strong communication skills and an empathetic nature to work collaboratively in a team environment.

Location: Viewcrest Health Center
Schedule: Full time, hourly position
Assist residents with social, emotional, and care-related needs
Support the Social Services team in developing care plans
Advocate for residents’ rights and well-being
Coordinate resources and referrals for residents and families
Foster a warm, welcoming community
Experience in social work, healthcare, or a related field (preferred)
Strong communication and interpersonal skills
A caring and empathetic nature
Ability to work collaboratively in a team environment
