Coordinate paid and non-paid services for individuals with intellectual and developmental disabilities to help them live fulfilling community lives. Create individualized service plans and monitor documentation to ensure regulatory standards and personal goals are met.
Requires a bachelor's degree and at least one year of experience working with people with intellectual or developmental disabilities. Must possess a valid driver's license, an insured vehicle, and the ability to travel throughout New Jersey.

Values Into Action- NJ is hiring a Support Coordinator to join our team. As a Support Coordinator, you will assist individuals with intellectual and/or developmental disabilities to coordinate both paid and non-paid services and supports that will aid them in living a fulfilling life within their community. This includes working with the service user and their team to create an individualized service plan that clearly includes the person’s aspirations and goals; and on-going monitoring of services and completion of service documentation to ensure that the individuals’ needs are being addressed and regulatory standards met.
This position offers flexibility with some tasks that can be completed remotely. However, it also requires regular engagement in the community and travel within the region to meet the needs of those supported and required by our funders. The ideal candidate will live in Mercer County of New Jersey. Frequent travel throughout the state of New Jersey is expected in this role.
