Position Summary
The Nurse Practitioner or Physician Assistant provides comprehensive primary care services for participants enrolled in the Program of All-Inclusive Care for the Elderly (PACE). Working collaboratively with the Medical Director and Interdisciplinary Team (IDT), this role assesses, diagnoses, treats, and manages acute and chronic medical conditions while supporting participants' goals of remaining safely in their homes and communities whenever possible.
The APP participates in care planning, preventive health initiatives, transitions of care, and ongoing management of medically complex older adults in accordance with CMS regulations, state practice requirements, and organizational policies.
Qualifications
Required
Graduate of an accredited Nurse Practitioner or Physician Assistant program. Current Oklahoma NP or PA license in good standing. Current DEA registration (or ability to obtain). Current BLS certification. Eligible for Medicare and Medicaid credentialing. Valid driver's license with reliable transportation for community visits.
Preferred
One (1) year of experience working with frail or elderly adults, or completion of approved training in the care of frail older adults as required under PACE regulations. Primary Care, Geriatrics, Internal Medicine, Family Practice, or related clinical experience. Experience working within an interdisciplinary healthcare team.
