Transitional Support Specialist - 2-Days

Full-time, Pay Range: $18.28 - $21.78 - Various Locations

UCP Seguin of Greater Chicago is seeking compassionate, reliable, and dedicated individuals to join our team as a Transitional Support Specialist (TSS). This position offers a unique 2-day work schedule consisting of two 24-hour shifts per week (48 hours total), allowing employees to work full-time while enjoying additional days off. The Transitional Support Specialist provides direct support, personal care, and skill development services to adults with developmental and other disabilities, helping them achieve greater independence and meaningful participation in their communities.

Qualifications:

• Valid Illinois Driver's License required.
• Proof of current automobile insurance in accordance with Illinois state requirements.
• Reliable personal vehicle available for work-related responsibilities.
• Ability and willingness to transport participants safely to appointments, community activities, employment sites, and programs.
• Ability to obtain and maintain required certifications, including:
  • CPR and First Aid
  • Medication Administration
  • Defensive Driving
  • Behavior Management Training
• Strong communication and interpersonal skills with the ability to work effectively with participants, families, healthcare professionals, and team members.
• Ability to maintain confidentiality and uphold participant rights at all times.
• Commitment to safety, professionalism, and providing high-quality support services.

Key Responsibilities:

As a Transitional Support Specialist, you will assist participants with daily living activities such as personal care, meal preparation, medication administration, housekeeping, mobility support, and community engagement. You will help individuals develop social, vocational, communication, and independent living skills while supporting their participation in recreational activities, volunteer opportunities, employment programs, and community events. The role also includes maintaining accurate documentation, implementing behavior support strategies, participating in team meetings, and ensuring the health, safety, and well-being of all participants.

Work Schedule:

Typically, a two-day, 48-hour shift with five hours of sleep time per day; the schedule may vary by program needs.
