The provider will deliver personalized, coordinated healthcare to patients in nursing homes and assisted living facilities to improve outcomes and prevent re-hospitalization. Responsibilities include assessing medical needs, prescribing medications, ordering diagnostic tests, and ensuring compliance with clinical regulations.
Candidates must hold an active APRN or PA license and a degree from an accredited program. A minimum of 3 years of clinical experience in geriatric nursing or rehabilitation is required.

You will be responsible for providing our patients with personalized, coordinated health care that improves patient outcomes and prevents re hospitalization.
As one of our advance practice providers, you will exercise your independent judgement to treat patients with acute and chronic conditions, consulting with a supervising physician as appropriate. You will work closely with our facility partners and health plan leadership to Implement our Treat in Place Protocols in compliance with all federal and state regulations.
