The RN Case Manager serves as the clinical anchor, coordinating care and guiding interdisciplinary teams for hospice patients. Responsibilities include conducting comprehensive assessments, adjusting personalized care plans, and educating patients and caregivers.
Candidates must hold an active RN license and be a graduate of an accredited nursing school. Preference is given to those with over one year of experience in hospice, home health, or related organizations.

As an RN Case Manager, you'll be the clinical anchor for your patients and their families — coordinating care, guiding the interdisciplinary team, and showing up when it matters most. Your day-to-day includes:
