The Psychiatric Rehabilitation Services Aide provides group and individual psychosocial rehabilitation and case management services to adults in a long-term care setting. They collaborate with an interdisciplinary team to develop, implement, and evaluate therapeutic programming for residents.
A high school diploma or GED equivalent is required for this position. Prior experience as a Psychiatric Rehabilitation Services Aide in a licensed nursing home is preferred.

What you will do in this role: The Psychiatric Rehabilitation Services Aide (PRSA) is responsible for group and individual psychosocial rehabilitation and case management services to adults within a long term care/nursing home setting. The PRSA will participate with the interdisciplinary team to develop, implement and evaluate effective therapeutic services via facility programming.

This center is managed by TRANSITIONAL CARE MANAGEMENT. We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.
