Full job description
The community health worker (CHW) will provide access to preventive and medical care services for patients in the ambulatory and in-patient settings. CHWs provide direct linkage to supportive resources which address and better support social determinants of health.
Provides screenings of risk-factors that impact an individuals access to care
Follows up with patients by telephone to connect to medical appointment's and answer questions about the recommended next steps
Builds relationships with patients by telephone to connect to medical appointment's and answer questions abut the recommended next steps
Builds relationships and maintains contact with existing business partners that support business office functions
Follows up and assists families who are having difficulty accessing resources to which they were referred
Fully understands and is able to explain the issues for which they are screening and the resources available to meet those needs
Understands and can explain insurance and CalFresh programs (such as eligibility, benefits, application process, requirements, premiums, and co-pays) and other community health services
