Conduct comprehensive in-home assessments and develop person-centered service plans for older adults and individuals with disabilities. Coordinate long-term services through the MyCare Ohio Waiver Program while ensuring compliance with Medicaid regulations.
Requires an active Ohio LSW or RN license and at least one year of experience in home care, disability services, or case management. Must possess a valid driver's license, reliable transportation, and the ability to obtain Ohio Department of Aging certification.

Care Manager (also known as a Waiver Service Coordinator) is responsible for conducting in-home assessments, identifying support systems, and coordinating long-term services through the MyCare Ohio Waiver Program. This role ensures members receive appropriate care in home- and community-based settings while maintaining compliance with Medicaid and program regulations.
This position is primarily office-based in a climate-controlled environment with regular in-home visits. Requires extended computer use and occasional lifting to 25 lbs.
Hiring is contingent upon successful completion of a drug screen, background check, and reference check. AOoA is a drug-free workplace. Employment is contingent upon funding availability.
