Facilitates comprehensive care for stroke patients by coordinating the continuity of care from admission to discharge through collaboration with interdisciplinary team members. The Stroke Program Coordinator facilitates the implementation of the Stroke Care Guidelines and other evidence/research-based practice and ensures that Hospital's Stroke Program incorporates the recommendations for Primary Stroke Centers published by the Brain Attack Coalition and the American Stroke Association.

Experience 3 years experience in adult health with a focus on care of the stroke patient throughout the stroke continuum required.
