The Integrated Family Nurse Practitioner provides comprehensive, patient-centered primary care by diagnosing and treating acute and chronic conditions within an interdisciplinary team. They are responsible for patient education, chronic disease management, and maintaining accurate documentation in the electronic health record.
Candidates must hold a Master's or Doctoral degree in Nursing and be licensed as an Advanced Practice Registered Nurse in Texas with national Family Nurse Practitioner certification. Strong clinical decision-making skills and current Basic Life Support certification are required, with experience in community health settings preferred.
Position Summary: The Integrative Nurse Practitioner (NP) provides comprehensive, patient-centered primary care services within TAN Healthcare's integrated care model. The NP evaluates, diagnoses, and treats acute and chronic medical conditions while emphasizing disease prevention, health promotion, and patient education. Working collaboratively with physicians, behavioral health professionals, care managers, nursing staff, and other interdisciplinary team members, the Integrative NP delivers whole-person care that addresses the physical, behavioral, and social needs of patients.
