The LPN provides remote on-call triage services by assessing patient symptoms and coordinating care plans according to established protocols. They also manage documentation, perform chart reviews, and facilitate communication between patients, physicians, and acute care settings.
Candidates must hold a current compact state LPN license and possess at least one year of nursing experience. Experience in home health or triage is highly preferred, along with a degree or certification from an accredited nursing school.

*Monday, Friday, Saturday & Sunday, 10p-5a CST (11p-6p EST)
*Wednesday, Saturday & Sunday, 10a-6p CST (11a-7p EST)
*Friday, Saturday & Sunday, 10a-6p CST (11a-7p EST)
*Friday, 4p-8p CST (5p-9p EST) and Saturday & Sunday 8a-4p CST (9a-5p EST)
This LPN provides On-Call and Triage for the assigned population in accordance with triage protocols. This position remotely develops/implements care plans for persons served in the area of responsibility. The Tele-Triage LPN assesses health status, identifies problems and needs of person(s) served in their assigned caseload, develops nursing care plans in collaboration with operations LPN’s. You will complete chart reviews and monthly host home reviews and perform On-Bound calling to assigned populations/locations. This LPN performs data analysis in multiple systems.
