Job DetailsJob Location: Santa Rosa - Santa Rosa, CA 95401Position Type: Full TimeEducation Level: 4 Year DegreeSalary Range: $222,061.00 - $305,129.00 SalaryJob Shift: DayJob Category: Health CarePOSITION SUMMARY: The Family Practice Physician reports to the Chief Medical Officer. The purpose of this position is to perform duties of a medical physician and abide by the physician’s oath when providing care to SCIHP patients in a team based, collaborative, patient centered medical home.

QualificationsESSENTIAL FUNCTIONS

Take appropriate history, perform physical examination of patients, order relevant diagnostics and deliver appropriate therapy according to standard medical practice.
Referral for examination and consultation to other providers when problems are unfamiliar or outside scope of practice.
Supervision of PA’s and NP’s according to the standardized procedures defining their scope of practice.
Some training of support staff.
Rotate on-call duties during the week and on weekends with other providers.
Demonstrate competent knowledge in delivering health care to patients through peer review, participation in Quality Improvement Program, and collaboration with peers in case review/management panels.
Assure quality and safety by adhering to PCMH principles, including teamwork, evidence-based medicine, CQI, and active participation of patients in decision making.
Nursing home visits on agencies patients.
Punctual participation in departmental, organizational meetings, and committees as assigned.
Maintain legible, accurate, and timely completed notes on clinic patients in S.O.A.P. style in the Electronic Health Record.
Maintain problem lists, medication profiles and health maintenance on all clinic patients.
May participate in outreach clinics to Kashia or Point Arena.
Respectful, timely and collegial communication with all staff.
Demonstrate exemplary professionalism and cultural sensitivity.
Maintain credentialing by keeping all required licenses current, including but not limited to, California Medical License, DEA License, CME hours, CPR Certifications and any other required licenses.

EDUCATION AND EXPERIENCE:

A current License by the Medical Board of the State of California. Hold a current Controlled Substance Registration Certificate (DEA).
Current CPR certificate.
Eligible for hospital privileges.

QUALIFICATIONS:

Knowledge of modern methods, theories, techniques and procedures used in medicine.
Knowledge of requirements used for Quality Assurance.
Knowledge of principles, techniques and best practices related to team based primary care models.
Knowledge of function services provided by other SCIHP departments and the ability to work cooperatively with those departments.
Knowledge of related accreditation and certification requirements, strong leadership skills and ability to conceive, plan and execute projects and programs as well as supervise, lead and develop staff.

SPECIAL QUALIFICATIONS:

All employees must be sensitive to the needs of the Native American community, demonstrate a deep understanding and respect for the diverse cultural heritage, traditions, and historical context of the Native American community.

Sonoma County Indian Health Project, Inc. (SCIHP) was established in 1971 by a group of leaders from the Indian communities of Sonoma County. Their goal was to provide health care for all Indians of Sonoma County and to provide services in a manner which is sensitive to the culture and traditions of the local Indian Tribes. Over the years, SCIHP has steadily progressed from a small organization consisting of a single dental chair and an exam table to the multimillion-dollar comprehensive health care facility it is today. In 1999, the Board of Directors acquired private land. In collaboration with the Tribes, SCIHP secured several grants through HUD (Housing and Urban Development) to begin building our Facility. Our initial operation revenue was through the Indian Health Service, under the Indian Self-Determination and Education Assistance Act (P.L. 93-638). In 2001, we were able to move into our newly built facility, which is where we are presently located. SCIHP is funded by State, County and private funds. Through a special relationship with the California Rural Indian Health Board (CRIHB), SCIHP is sub-contracted with the federal government. In effect, this establishes SCIHP as a tribal organization under the provisions of P.L. 93-638, and as such, performs the functions of the Indian Health Service throughout our assigned service area. This relationship carries certain responsibilities to the tribal governments, which comprise our contractual relationship. SCIHP is licensed by the State of California as A Community Health Center. We currently provide medical, dental, nutritional, behavioral health, pharmacy and health education services. SCIHP has expanded care to non-Indians with Medi-Cal on a limited basis.
