Established in 1978, SWCHC provides a wide range of health care services such as primary health care, health promotion, community development, mental health, and harm reduction. SWCHC is well known for its excellent Community Health Centre (CHC) model and its professional health services team, but it has also become a leader in offering a wide range of community-building activities and advocacy programs. What You’ll Do
Reporting to the Manager of Primary Health Care the Clinic Nurse works as part of an interdisciplinary team to provide coordinated care with other health professionals and support the overall function of the clinic.

What We’re Looking For
◈ Life and Work Experience
- Three to five years’ experience in community nursing working with diverse populations
- Previous clinic experience an asset
- Previous experience using an EMR, preferably Practice Solutions Suite (PSS)
- Experience providing routine immunizations for all ages
◈ Your Education Path
- Current license with the College of Nurses of Ontario (RN/RPN)
- Current CPR (preferred)
◈ Required Language
- Fluency in English required
- Working knowledge of French or other languages an asset
What You Bring to the Table
- Knowledge of primary health care and the social determinants of health
- Able to work effectively in an inter-professional environment
- Knowledge and awareness related to cultural competence
- Experience working with disadvantaged communities
- Knowledge of nursing care related to Primary Care/Community Health/Chronic Disease Management

Your Day-to-Day at a Glance
As our Clinic Nurse, your responsibilities include, but are not limited to:
- Nursing assessments
- Telephone & in-person triage
- Administer medications & treatments
- Client education
- Perform procedures within the scope of practice such as wound care, ear syringing
- Client advocacy
