At Sangre de Cristo Community Care, our Admissions Coordinators are often the very first voice our patients and referral partners hear. This role is fast-paced, mission-driven, and incredibly rewarding. Every referral represents a patient and family who need our help quickly, and you'll play a key role in ensuring they receive seamless, compassionate care from the very beginning.

As an Admissions Coordinator, you'll be at the center of our referral process, coordinating multiple moving pieces while ensuring patients receive timely access to care.
