Candidates must hold a current unencumbered Registered Nurse license and have at least one year of experience in a medical-surgical or acute care setting. Requirements include CPR certification, computer proficiency with EMR, and a valid driver's license with dependable transportation.

Sanctuary Hospice is seeking a Per Diem Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.
Schedule: Per Diem, Monday - Friday, 8 am - 4:30 pm
Territory: Wheaton, Warrenville, and Lisle areas.
In this rewarding role, you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. As a valued member of our interdisciplinary care team, you’ll help ensure every patient receives The Best Care Possible — care grounded in compassion, clinical excellence, and connection.
