The Health Navigator manages care for unhoused individuals by facilitating access to services and maintaining ongoing contact within homeless encampments. They perform comprehensive barrier assessments and assist participants in enrolling in public benefit programs and healthcare services.
Candidates must have experience in Substance Use Disorder counseling and a valid California Driver License. Preference is given to those with a personal history of systems involvement or two years of experience in non-profit or community clinic settings.

The Health Navigator serves as the primary care manager for unhoused individuals and families who need services. Working alongside the street medicine team in homeless encampments, they facilitate the initial introduction to the organization’s services and maintain ongoing contact with each assigned participant. This position will work with individuals who have substance use disorder.
