Position Summary
Under the administrative and clinical supervision of the Medical Director through a written supervision agreement, performs various patient care, physician support duties and tasks as required.
Salary Range: $110,000 - $150,000
Actual pay will be determined based on several factors. These may include education, work experience and in some instances, certifications. We strive for market alignment and internal equity with our colleagues’ pay.

Essential Functions
- Provides Mental Health and Addiction treatment services to patients in Middletown, CT clinic.
- Provides continuity of care by arranging follow-up appointments, coordinating care with a team of social workers and counselors.
- Provides care services including, but not limited to, initial evaluation, physical examination, and medication management, coordinating care with other providers, ordering labs and EKGs and obtaining information on adverse events.
- Provides continuity of care by arranging follow-up appointments.
- Administers medications, including but not limited to Esketamine (Spravato).
- Obtains prior authorization for services, provides assessments and other testing as needed.
- Prepares reports and participates in agency quality assurance activities.
Qualification Requirements
Current State of Connecticut certification and/or license as an Advanced Practice Registered Nurse. Active DEA controlled substance registration and an active Connecticut controlled substance registration. DATA 2000 Suboxone waiver, or willing to obtain one.
