Provides case management services to children and adolescents with intellectual and developmental disabilities in the community. Assists families with linking to health, financial, educational, and social services while maintaining accurate treatment documentation.
Requires a bachelor's degree in a human services discipline and one to three years of relevant experience. Candidates must possess excellent interpersonal skills and have a reliable means of transportation for home visits.
