Provide outreach, housing transition, and short-term sustaining services to individuals with serious mental illness or substance use disorders. Coordinate with property managers and community providers to ensure stable housing and connection to long-term support services.
Requires a Bachelor's degree in behavioral science or a related field and 2,000 hours of supervised clinical experience. Candidates must be able to transport clients in the community and maintain contemporaneous clinical documentation.

Starting Salary Range: $57,437 + Office Benefit Package (Parking/Remote Expenses): $180/Month subject to change
This position will provide professional, quality program services to the individuals served by the Section 811 Program. The Mental Health Practitioner will provide Housing Support Services to people with serious mental illness and/or substance use disorders that are enrolled in the Section 811 program in the Metro area. The primary function of the practitioner is to provide outreach, housing transition, and short-term housing sustaining services, with the goal of connecting the person to ongoing services.
Hours: Monday - Friday 8 am - 4 pm or 9 am - 5 pm, 40 hours per week
