Philadelphia FIGHT is seeking a licensed RN to join our excellent crew of providers as our Nurse Care Coordinator! Our Nurse Care Coordinators assess patients, provide direct patient care, coordinate care with other healthcare professionals and advocate for patient rights. In addition, Nurse Care Coordinators provide patients with health education, ensuring patients understand their health conditions, treatment options and care strategies.

About the Role:

  • Schedule: Full-time, Monday - Friday (9:00AM - 5:00PM)
  • Location: Philadelphia FIGHT Community Health Centers, 19107
  • Compensation & Benefits:
    • FSLA status is exempt with an annual rate of $103,500K 
    • 3 weeks of PTO (4 weeks after 1st year) + 11 paid holidays

Requirements:

  1. Bachelor's degree in nursing required.
  2. 3+ years' experience in nursing, preferably in an outpatient primary care medical setting/FQHC.
  3. Must possess basic computer skills to perform job duties including desktop computing, electronic medical records, e-mail, time sheet management, reporting, internet research, etc. using relevant software such as HRIS and Microsoft Word, Excel, etc.
  4. Must possess CPR certification or be willing to be certified within 60 days of employment.
  5. Demonstrate the ability to understand the overall purpose of clinical care and importance of support staff.
  6. Discretion, initiative, and ability to maintain HIPAA compliance.
  7. Due to a high volume of Spanish speaking patients, bilingual candidates (Spanish/English) are strongly preferred.
  8. Must be able to exercise good judgment and identify when to escalate issues as appropriate.
  9. Knowledge of de-escalation methods or willingness to be trained in these methods.
  10. Ability to handle stressful situations while providing exceptional customer service.
  11. Must be able to demonstrate judgment, patience, integrity, organization, and prioritization in a multi-task environment.
  12. Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices such as OSHA, HIPAA, HRSA, CLIA and funding body regulations.
  13. Strong skillsets in interpersonal relationships and teamwork, attention to detail, dependability, customer service, time management, autonomy and problem solving.
  14. Candidates must possess strong communication skills, the ability to multi-task and excellent attention to detail and follow-up skills.
  15. Ability to remain solution-focused and respectful in all interactions with staff, co- workers, vendors, and patients.
  16. Excellent problem-solving and conflict resolution skills.
  17. Ability to display a non-judgmental attitude.

Responsibilities:

  1. Conducts comprehensive assessments of patients' health status, including physical, psychological, and social factors; collects vital signs, medical history, and symptoms to identify health problems and develop care plans.
  2. Provides hands-on care, including administering medications, performing medical procedures, monitoring vital signs, managing wounds, and assisting with daily living activities; triages sick/emergency walk-ins/calls.
  3. Collaborates with a full range of other healthcare professionals, such as physicians, behavioral health consultants, PrEP staff and medical case management to ensure that high risk patients are meeting their treatment plan goals.
  4. Provides one-on-one education regarding diagnosis, treatment options and self-care strategies to patients with multiple diagnoses, complicated medical regimens, and/or complex social needs; promotes health and prevents illness by providing information on healthy lifestyle choices and disease prevention.
  5. Informs patients of lab results (normal and abnormal) and recommends follow-up based on medical provider recommendation; advocates for patients' needs to ensure continuity and quality of care.
  6. Completes all charting and documentation in electronic medical records (EMR) system as needed and required by contracted funding sources and billing, including assessments, progress notes, billing codes and administrative documentation.
  7. Coordinates ordering of clinical supplies, including monitoring for expiration of supplies and maintenance of clinical medical equipment (e.g., medical supplies and prescriptions).
  8. Monitors tracking board to ensure patient flow and cycle times throughout the clinic.
  9. Acts as a back-up to Medical Assistants as it pertains to therapeutic injections, vaccinations, phlebotomy, EKG, oxygen setup, blood glucose monitoring, urine dipsticks and pregnancy testing, etc.
  10. Assists medical providers in the completion of essential clinical paperwork and in the generation of letters of medical necessity.
  11. Advocates for assigned patients, ensuring that their rights and preferences are respected; ensures that patients receive the best possible care and treatment.
  12. Monitors and reconciles orders in the EMR and CLIA related tasks such as running of controls as needed.
  13. Reports and assists in communicating appropriate information on reportable diseases to oversight bodies, e.g., Public Health Department, AIDS Activities Coordination Office.
  14. Collaborates with medical records to assist providers with obtaining urgent reports from diagnostic exams, specialist consultations, and hospitalizations as needed.
  15. In conjunction with medical assistants, assists providers and/or nurse managers, in the scheduling of imaging studies, procedures and specialty appointments, obtaining authorization for off-formulary medications, particularly those requiring prior authorization, as needed.
  16. If working in Pediatrics unit, oversees the practice's Vaccine for Children Program as the VFC Coordinators.
  17. If working in Pediatrics unit, organizes a daily clinical huddle with providers, nurses, medical assistants and other support staff.
  18. Participates actively in Quality Improvement initiatives and committees.
  19. Participates in regularly scheduled practice-related meetings and attend FIGHT case conference weekly as needed.
  20. Documents and notifies leadership of incidents and/or patient complaints.
  21. Upholds and ensures compliance and attention to all FIGHT policies and procedures as well as the overall mission and values of the organization.
  22. Other duties as assigned to fulfill FIGHT's mission.
