Care Managers support youth and families through a collaborative, strength-based Wraparound approach. They facilitate meetings, provide assessments, and coordinate services across multiple systems.
Candidates must have a bachelor's degree in a related field and at least one year of relevant experience. Bilingual candidates are encouraged to apply, and knowledge of the Essex County community is preferred.
