The APP provides holistic assessments and manages symptoms to improve the quality of life for patients with serious illnesses. They guide families through complex medical decisions and coordinate care transitions within an interdisciplinary team.
Candidates must be a licensed Nurse Practitioner or Physician Assistant with a relevant advanced degree (MSN, DNP, or MMSc/MPAS). Preference is given to those with at least one year of experience in palliative care, geriatrics, or complex chronic disease management.
