The Senior Care Healthcare Liaison will build and maintain referral relationships with healthcare providers and community partners across the Twin Cities. They will serve as a trusted resource for care teams and represent Oasis at professional events to ensure seamless patient transitions.
Candidates must have at least 2 years of experience in business development or community outreach within the hospice, home health, or senior living sectors. A deep understanding of the continuum of care and existing professional relationships within the Minneapolis healthcare landscape is required.
