Someone with your depth of compassion, clinical insight and strong decision-making capabilities is the ideal person to map out and oversee the process of a patient’s recovery from illness, injury or surgery at home. At MJHS, our dedicated staff provides a full range of advanced clinical home care services, including patient assessment, medication management and patient/family education.
In this role, you will evaluate the home care needs of our patients. You will also review patient insurance information, coordinate activities involved in each individual care plan and make sure that pertinent findings are shared with family and professional caregivers. Your excellent communication, documentation and time management capabilities will be essential to your success. And all along the way, you will experience the respect for your opinions and the high level of autonomy you need to do the job right.
Qualifications
Excellent clinical assessment skills
Strong ability to solve problems independently and interact with an integrated team
Current NYS RN license and registration
Bachelor’s degree in nursing preferred
Minimum of one year acute medical-surgical nursing experience
