The After-Hours Admissions Nurse conducts in-person nursing visits to assess, plan, and provide end-of-life care for hospice patients. They are responsible for effective symptom management and coordinating services with the transdisciplinary team and outside providers.
Candidates must hold a current California Registered Nurse license and possess an Associate's or Bachelor's degree in nursing. Experience in acute care, home health, or hospice settings is preferred.

We are currently seeking an RN to be a part of our team. The ideal candidate will provide coverage for After-Hours to cover patient/staffing needs and processes admissions for eligible hospice patients in coordination with Team Manager.
Schedule:
Monday - Friday.; 4:00pm-12:30am.
Location:
Alameda County with additional coverage to San Mateo County as needed
