The Hospice RN Case Manager is responsible for assessing, planning, and implementing supportive care for a patient/family caseload. This includes coordinating with physicians and other hospice staff to ensure optimal end-of-life care and resource utilization.
Candidates must hold a current California Registered Professional Nurse license and a valid driver's license with insurance. A Bachelor's degree in Nursing or one year of professional nursing experience within the last three years is required.

We are seeking a Hospice Nurse Case Manager to be responsible for identifying patient/family needs and for providing skilled nursing assessment, planning and supportive care and appropriate services in accordance with the attending physician’s plan of treatment and orders.
Schedule:
Monday - Friday; 8:30am - 5:00pm
