The Patient Care Coordinator serves as the primary point of contact for families, therapists, and physicians to ensure efficient coordination of pediatric home health therapy services. This role manages documentation, insurance authorizations, and physician orders while maintaining accurate patient records in the EMR system.
Candidates must be bilingual in English and Spanish with strong organizational, communication, and customer service skills. Proficiency in Microsoft Office and experience in healthcare administration or home health settings are highly preferred.

This role serves as the primary point of contact for families, therapists, physicians, and referral partners after therapy services are established, helping ensure exceptional customer service, timely communication, and efficient coordination of care.
The ideal candidate is highly organized, detail-oriented, and passionate about providing outstanding service while managing documentation, authorizations, and daily operational workflows in a fast-paced healthcare environment.
