As a Care Coordinator Social Worker, you will be responsible for exemplary cross continuum care coordination, including psychosocial assessments and interventions. You will collaborate with physicians, peers, and community colleagues to improve patient care and services.
Candidates must have at least 5 years of recent clinical experience in a healthcare setting and a Master's Degree in Social Work from an accredited program. A Licensed Social Worker certification is strongly preferred.

Position: Care Coordinator MSW
Shift: Per Diem, Varied Shifts; mainly weekends, some weekdays to cover. 16 hours/month + one weekend per month + 1 holiday per year
Experience:
1. At least 5 years of recent clinical experience in a physician’s office, clinic, hospital, or emergency services (EMS) setting, or other equivalent experience.
2. Leadership experience a plus.
Education:
Master’s Degree in Social Work from CSWE (Council on Social Work Education) accredited program.
Licensures/Certifications:
Licensed Social Worker strongly preferred.
