The RN Case Manager provides hands-on end-of-life care and coordinates services for hospice patients and their families. Responsibilities include assessing client eligibility, developing personalized care plans, and participating in on-call duties.
Candidates must hold a current unencumbered Registered Nurse license and have at least one year of experience in a medical-surgical or acute care setting. Additional requirements include CPR certification, a valid driver's license, and proficiency with EMR software.

Schedule: Monday - Friday, 8 am - 4:30 pm
Territory: Fishersville
In this rewarding role, you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. As a valued member of our interdisciplinary care team, you’ll help ensure every patient receives The Best Care Possible — care grounded in compassion, clinical excellence, and connection.
