The RN Case Manager guides medically complex patients through interdisciplinary care plans from admission to safe community discharge. Responsibilities include conducting utilization reviews, performing biopsychosocial assessments, and collaborating with physicians and external agencies to ensure smooth transitions.
Candidates must hold an active Missouri RN license, BLS certification, and have 1–3 years of experience in case management or discharge planning. An Associate's or Bachelor's degree in Nursing is required, with a BSN preferred.
