Order medications, diagnostic studies, and therapeutic procedures related to their illnesses and trackresultstimelyas outlined. The Transplant Coordinator III-NP alsoassistsother personnel to ensurehigh qualitydelivery of patient care and services to a transplant/end stage organ disease patient population. Develops and coordinates a system to organize the flow of patients frominitialreferral, through evaluation and education, to post-transplant follow-up andreferralback to the community. Participates in collaborative, interdisciplinary research activities related to transplant patients, processimprovementorevidencedbased practice activities. Attend professional meetings and conferences,maintainprofessionalaffiliationsand enhance professional development tomaintainknowledge of advancesoftransplant populations and end-stage organ diseases. Performs other special projects and duties as assigned. This position may require rotating on-call responsibilities as needed, including nights,weekendsand holidays. Essential Duties:

The annual base salary range for this position is $145,600.00 - $237,120.00.
