The Renal Care Coordinator organizes care for late-stage chronic kidney disease patients by providing education and support for treatment transitions. They act as a liaison between patients and the nephrology practice to ensure comprehensive understanding of dialysis modalities and kidney health.
Candidates must have a minimum of 2 years of experience in clinical renal patient care, including work with transplant, dialysis, or CKD patients. Strong organizational and communication skills are required to effectively manage patient populations and clinical processes.
